关于上海市血友病人HIV病毒感染者要求更换毒副作用较小的抗病毒药物的情况说明及请求

上海市人民政府领导:

 

    我们是因使用上海生物制品研究所生产的血制品感染艾滋病毒的上海市血友病患者,近些年来在上海市政府、卫生局、上海市公共卫生中心(原上海市传染病医院)医务人员关心和精心诊治下,绝大多数患者的生命得到了保全,但由于我们目前服用的抗HIV药物毒副反应日益显现,严重影响了我们的身心健康与生命安危。现将我们使用抗HIV病毒药物的毒副反应及我们的诉请说明如下:

 

    自2001年12月起上海血友病HIV感染者开始陆续使用抗病毒药物(基本为3TC+D4T+EFV,个别为AZT+D4T+EFV)以来已有近四年,多数病人服药已超过三年,目前部分病人已出现了耐药倾向。更为严重的是,在这些年中已有两位病人因抗病毒治疗的毒副反应而致死亡,同时所有接受抗病毒治疗的病人都出现了普遍的,严重的毒副反应,必须引起有关职能部门的高度重视!现详细列表如下:

    鉴于以上抗病毒治疗的毒副反应及耐药倾向日益显现,撤下毒副作用严重的药物,如D4T等,替换上毒副作用较小的新型抗病毒药物已经刻不容缓!目前国际上共有四大类(NRTIs)、(NNTIs) 、(PIs) 、(FIs),共24种抗HIV病毒药物(经美国FDA批准,截止2004年12月)(见附表一),而目前我国内现有的抗HIV病毒药物(含已注册)仅为三类12种,能够真正使用的仅有8种,四种药物只在我国进行了注册并未实际销售。(见附表二)

   
    虽然上海市血友病感染HIV患者都发生了相当严重的毒副反应,但此严重问题因没有引起有关部门足够重视,致使这些患者目前无法更换公认的、国内尚未进口、毒副作用相对较小的替诺富韦(tenofovir ,TDF)、Fuzeon(enfuvirtide,T-20)等新型抗病毒药物。由于血友病病人自身的特殊性基本不适用有加重出血倾向的蛋白酶抑制剂类药物(PIs)及齐多夫啶(AZT);同时由于合并感染HCV、HBV也不适用可导致严重肝病的去羟肌苷(DDI)与奈韦拉平(NVP)。因此对于血友病患者而言,国内现有的抗病毒药物都有可能造成更为严重的毒副反应,显然已不能继续使用下去。为了确保生命与健康,我们迫切需要新的抗病毒药物组合以满足血友病患者的进一步治疗诉求。

   
    综上所述,我们服用的抗病毒药物已经严重损害了我们的身心健康,严重威胁了我们的生命安全。而目前国内尚无适合广大血友病患者的新药物组合可供替换。为此,我们强烈要求尽快使用目前国际上相对适合血友病患者的抗病毒药物替诺富韦(tenofovir,TDF), Fuzeon(enfuvirtide,T-20)等,并在药物组合的多样性上尽早与国际接轨,以降低死亡率,让我们这些不幸的患者健康、和谐、愉快地融入社会主义大家庭中。因此,我们郑重提出我们的请求,恳请上海市政府配合国家有关部门尽快解决上述药物的进口事宜,更好地为众多血友病感染者造福。

If taken with ddI (Videx or Videx EC), Aptivus/Norvir should be taken at least two hours before or two hours after taking ddI.

Crixivan® (indinavir), by Merck & Co.

Two 400mg capsules, every 8 hours (a total of 6 pills a day), or two 400mg Crixivan capsules with either one or two 100mg Norvir capsules twice a day (preferred dosing).

Invirase® (saquinavir), by Hoffmann-La Roche

Invirase must be used in combination with Norvir. The approved dose is two 500mg Invirase tablets plus one 100mg Norvir capsule, twice a day (a total of 6 pills a day).

Invirase and Norvir should be taken with food, preferably a full nutritious meal (e.g., breakfast and dinner), or within 2 hours after a meal.

Kaletra® (lopinavir + ritonavir), by Abbott Laboratories

Three capsules, twice a day (a total of 6 pills a day), or, if starting therapy for the first time, six capsules once a day. Each capsule contains 133.3mg lopinavir + 33.3mg ritonavir.

Diarrhea (common); nausea; feeling week/tired; headache
More rarely: Pancreatitis

Take with a meal or light snack. If combining with Sustiva® (efavirenz) or Viramune® (nevirapine), dose should be increased to 4 capsules, twice a day (a total of 8 pills a day). Should refrigerate for long-term storage (greater than 2 months), otherwise, room temperature storage is okay.

Lexiva® (fosamprenavir), by GlaxoSmithKline

Two 700mg tablets, twice a day (a total of 4 pills a day), or two 700mg tablets plus two 100mg Norvir capsules, once a day (a total of 4 pills a day), or one 700mg tablet plus one 100mg Norvir capsule twice a day (a total of 4 pills a day). This last dosing option should be used for patients who have tried and failed other protease inhibitors in the past.

Take with or without food. Patients who have previously taken a protease inhibitor should take one 700mg tablet plus one 100mg Norvir capsule twice a day (a total of 4 pills a day).

Norvir® (ritonavir), by Abbott Laboratories

Six 100mg capsules, twice a day* (a total of 12 pills a day). Start with 3 capsules, twice a day, and increase to full dose over 14 days. Note: the full dose of Norvir is rarely used any more. Norvir is most often used at much lower doses to "boost" the levels of other protease inhibitors in the body.

Take with food. High-fat snacks may reduce side effects. Capsules and oral solution may be stored at room temperature, but should be refrigerated in hot weather. Solution can be mixed with chocolate syrup, chocolate ice cream, or Ensure® to mask the taste.

Reyataz® (atazanavir), by Bristol-Myers Squibb

Two 200mg capsules, once a day (a total of 2 pills a day), or two 150mg capsules plus one 100mg Norvir capsule, once a day (a total of 3 pills a day).

Increased levels of bilirubin (a pigment found in the liver); headaches; pain/tingling in the arms and legs; nausea; diarrhea; rash

See long-term side effects below. Reyataz does not seem to negatively effect blood fat levels like other protease inhibitors.

Take with a light meal. For patients who have taken anti-HIV drugs in the past, the recommended dose is 300mg Reyataz plus 100mg Norvir (all as a single dose with food). This combination of Reyataz and Norvir is also recommended for patients who are using either Viread® (tenofovir) or Sustiva® (efavirenz) in a drug regimen that involves Reyataz.

Viracept® (nelfinavir), by Pfizer

Two 625mg tablets, two times a day (a total of 4 pills a day), or five 250mg tablets, twice a day, or three 250mg tablets, three times a day (a total of 9-10 pills a day).

Diarrhea (common); nausea; stomach discomfort; gas; rash; increased liver enzyme levels

Take with a meal or light snack. Your body absorbs the drug better with food in your stomach. If you have trouble swallowing the pills, there is a powder formulation that can be dissolved in water for easy drinking.

Long-term side effects of protease inhibitors: Use of protease inhibitors may be associated with changes in blood sugar levels (and rarely, development of diabetes), elevations in blood fat levels, and changes in the way the body stores fat (including development of fat deposits in the abdomen and on the back of the shoulders as well as loss of fat in the arms, legs and face) – see our lesson on Lipodystrophy. In addition, there have been reports of uncontrolled bleeding in hemophiliacs.

Take with or without food. Epivir is also used to treat patients infected with the Hepatitis B Virus (HBV), but at a different dose. If you are dually infected with HIV and HBV, the dosage indicated for HIV therapy should be used.

Epzicom™ (abacavir + 3TC), by GlaxoSmithKline

One tablet (contains 600mg abacavir + 300mg 3TC), once a day

Similar side effects to Epivir (3TC) and Ziagen (abacavir). Please note Ziagen's serious allergic reactions.

Retrovir® (AZT), by GlaxoSmithKline

One 300mg tablet, twice a day* (a total of 2 pills a day)

Nausea; stomach discomfort; headache; insomnia

Trizivir® (AZT + 3TC + abacavir), by GlaxoSmithKline

One tablet (contains 300mg AZT + 150mg 3TC + 300mg abacavir), twice a day (a total of 2 pills a day)

Similar side effects to Retrovir (AZT), Epivir (3TC), and Ziagen (abacavir). Please note Ziagen's serious allergic reactions.

Take with or without food. Do not take Trizivir if you weigh less than 90 pounds (40 kg).

Truvada® (Viread + Emtriva), by Gilead Sciences

One tablet (contains 300mg Viread + 200mg Emtriva), once a day

Similar side effects to Viread (tenofovir) and Emtriva (emtricitabine).

Videx® (ddI): buffered versions, by Bristol-Myers Squibb

Two 100mg tablets twice a day* (a total of 4 pills a day), or two 200mg tablets, once a day (a total of 2 pills a day). For patients weighing less than 133 lbs. (60 kg), click here.

Numbness, tingling, or pain in the hands or feet (peripheral neuropathy, seen in 15% of patients); nausea; diarrhea; vomiting; headache; rash
More rarely: Pancreatitis (inflammation of the pancreas)
Possible increased side effects if taken with tenofovir (Viread).

Clinical data supports twice-daily dosing as more effective. Take on an empty stomach (at least 2 hours after & 30 minutes before a meal). Okay to take at the same time as all NRTIs. The buffered versions of Videx should not be taken at the same time as any of the protease inhibitors (PIs); PIs should be taken two hours before or two hours after Videx. The NNRTIs Viramune and Sustiva can be taken at the same time as Videx; the NNRTI Rescriptor should be taken at least one hour before or one hour after Videx. Avoid alcohol.

Videx® EC (ddI): delayed-release capsules, by Bristol-Myers Squibb

One 400mg capsule once a day. For patients weighing less than 133 lbs. (60 kg), the dose is one 250mg capsule once a day.

Numbness, tingling, or pain in the hands or feet (peripheral neuropathy, seen in 15% of patients); nausea; diarrhea; vomiting; headache; rash

Viread® (tenofovir DF), by Gilead Sciences

One 300mg tablet once a day.

Nausea, vomiting, diarrhea, and flatulence (intestinal gas).
More rarely: osteopenia (thinning bones)

Take with or without food. If taken with ddI (Videx or Videx EC), it can increase ddI levels in the blood by as much as 60%, causing increased ddI side effects.

Zerit® (d4T), by Bristol-Myers Squibb

Ziagen® (abacavir), by GlaxoSmithKline

One 300mg tablet, twice a day* (a total of 2 pills a day)

Nausea; vomiting; diarrhea; loss of appetite; insomnia

Serious allergic reactions have been reported in about 8% of patients, with one death. The signs and symptoms of this reaction include fever, malaise, severe nausea, diarrhea, abdominal pain, sore throat, cough, shortness of breath, and rash. If allergic reaction is diagnosed, therapy should be discontinued and NOT restarted. Symptoms of allergic reactions usually appear within six weeks of starting therapy and resolve within 24-48 hours of stopping the drug. A rash without other signs of allergic reaction has also been reported in about 7% of patients. These rashes can usually be managed without stopping therapy.

Take with or without food.

Long-term side effects of NRTIs: NRTIs have been associated with damage to the mitochondria (the cell parts that provide energy to the cell). This damage may cause low red and white blood cell counts, muscle pain and wasting (particularly in the arms and legs), fatigue, peripheral neuropathy, and more rarely, serious liver (lactic acidosis) or pancreas problems. NRTIs have also been associated with changes in the way the body stores fat (including development of fat deposits in the abdomen and on the back of the shoulders as well as loss of fat in the arms, legs and face) – see our lesson on Lipodystrophy.

Take with or without food. Rescriptor can increase the blood levels of all of the available protease inhibitors. In turn, the dose of the protease inhibitor being used may need to be decreased. You should not take buffered Videx® (ddI) tablets – or anything containing an antacid – within one hour of a dose of Rescriptor. Both antacids and Videx tablets (which contain an antacid) can block Rescriptor from being absorbed into the bloodstream.

Sustiva® (efavirenz), by Bristol-Myers Squibb

One 600mg tablet once a day (just 1 pill a day)

Rash; drowsiness; insomnia; confusion; inability to concentrate; dizziness; vivid dreams; nausea; stomach discomfort; fever; insomnia; elevated liver enzymes. See Tips & Tricks on Taking Sustiva.

It is recommended that Sustiva be taken on an empty stomach. Dose should be taken at bedtime to minimize dizziness, drowsiness and impaired concentration. If Sustiva is taken with the protease inhibitors Reyataz, Invirase, Fortovase, Kaletra, Crixivan, Agenerase, or Lexiva, their doses may need to be increased or they may need to be combined with a low dose of Norvir (e.g., 100 mg) to help maintain necessary drug levels in the bloodstream.

Viramune® (nevirapine), by Boehringer Ingelheim

One 200mg tablet per day for 14 days, then one 200mg tablet, twice a day* (a total of 2 pills a day)

Rash; stomach upset; headaches; increased liver enzyme levels
More rarely: hepatitis

Take with or without food. Once daily dosing recommendation based on limited clinical data. If Viramune is taken with the protease inhibitors Reyataz, Invirase, Fortovase, Kaletra, Crixivan, Agenerase, or Lexiva, their doses may need to be increased or they may need to be combined with a low dose of Norvir (e.g., 100 mg) to help maintain necessary drug levels in the bloodstream.

Fuzeon® (T-20), by Trimeris and Hoffmann-La Roche

Two 90mg (in 1-ml solution) subcutaneous (under the skin) injections a day.

Reactions on the skin where Fuzeon is injected can include itching, swelling, redness, pain or tenderness, hardened skin, or bumps.
More rarely: serious allergic reactions

Fuzeon comes as a white powder that must be carefully mixed with in a vial with sterile water each day – a complicated process called "reconstitution".

4)血脂异常;

5)糖耐量降低,但极少出现糖尿病;

6)应用时间较长时可出现脂肪的重新分布。

由于RTV可引起较重的胃肠道不适,大多数患者无法耐受本药。故多作为其它PI类药物的激动剂,仅在极少的情况下单独。

(Kaletra)

LPV/RTV

蛋白酶抑制剂

成人:3粒/次,2次/日(Kaletra每粒含量:LPV 133.3mg,RTV 33.3mg)

儿童:7-15kg,LPV 12mg/kg和 RTV 3mg/kg,每天2次;LPV 15-40 kg     10 mg/kg和 RTV 2.5mg/kg,每天2次

主要为:腹泻、恶心、血脂异常;

也可出现 头痛和转氨酶升高。

与ddI合用时,ddI应在本药服用前1小时或服用后2小时再口服。
